Very promising, and the touted lack of side effects (mostly only G/I) and impact on negative symptoms is potentially game-changing. But I am sure many here have been down this road before.
If insurance doesn’t cover it, most people who need it won’t be able to afford $22,500 a year.
Lenkowsky says Bristol Myers Squibb is conducting a trial studying Cobenfy in combination with dopamine-based medications that will yield results in about a year.

I wonder if the combo would allow a person to take less of each. Reduce the dopamine receptor blockade drugs (I worry about dopamine supersensitivity) and add a smaller amount of Cobenfy if insurance does not cover it.
What has to happen for insurance to cover it?
Sorry, I don’t know what needs to happen for insurance to cover it. I would hope that Medicare would cover it. If not now, then in the future.
